Policies/Insurance

Arrival time: We ask that you come to your appointment 15 minutes early (for follow-up patients and new patients with all forms filled).  If you are a new patient and have not filled out our forms, please arrive 30 minutes early.

Insurance: Please bring your insurance card and/or prior-authorization form to each of your appointment.  You will also need your picture ID card (government issued).

Payment(s): All payments will be due at the time of check-in.  These payments include co-pays, deductibles, and co-insurance.   We accept cash, credit card, ATM, and money order/cashier’s check.  No personal checks will be accepted.

Insurance Claims: As a courtesy to you, our valued patient, we will file the insurance claim on your behalf for payment of service.  However, the full balance due is ultimately your (i.e. the patient’s) responsibility.

Prescription Refills: All medication refills will be made during your office visit.  Due to the nature of pain medications and for patient safety,  Dr. Lee will not accept phone call or pharmacy requests for refills.

Appointment Cancellations: Please call at least 24 hours before your appointment if you plan to cancel, and to avoid a “no-show” charge.  This applies to all appointments, including doctor’s office ($50) and surgery centers ($150).

Current Medications: Please bring your medications (e.g. especially pain medication bottles, boxes) to your office appointment.

Patient Feedback: Dr. Bryan Lee values all feedback from his patients regarding his practice.  Please do not hesitate to talk or write to him about your experience with him or his staff.  All comments will remain strictly confidential, unless patient instructs otherwise.
